At Theresa Shaver Orthodontics, our goal to provide you with affordable, high-quality orthodontic treatment that fits your budget. That's why we offer competitive pricing with a variety of payment options. When you come in for an initial exam, we will give you a comprehensive quote for treatment and review your insurance coverage, so that you aren't surprised with any additional costs or fees during treatment.

Payment Options

We offer a variety of payment options to meet your needs:

  • Automatic deduction – Monthly payments can be deducted automatically from your bank debit, credit card or checking account each month, on the day of your choice.
  • Credit cards – We accept American Express, Discover, Visa and MasterCard.
  • Checks – Mail-in payments are accepted and due the 1st of the month.
  • Interest-free payment plan – We offer a convenient interest-free payment plan based on the estimated length of treatment – with payments as low as $149 per month.

Insurance

We accept all major insurance plans. If you don't see your plan listed, please call our office, we continually add new insurance plans.

When you schedule an initial exam, we will gladly verify your insurance benefits and review them with you during your consultation.

As a convenience to our patients, we are happy to file insurance claims for treatment. We will also help you work with your company's Flexible Spending Account and provide any necessary paperwork you may need.

Fees & Financial Guidelines

In an ongoing effort to keep orthodontic treatment fees affordable and provide the highest level of professional care, we have established the following financial guidelines:

  • If payment is made in full at the start of treatment, we will offer a fee discount.
  • If you have insurance, if payment is made in full for your portion of the fee that will not be covered by insurance, we will offer a discount.
  • In case of serious delinquency, treatment will be discontinued, although we make every effort to accommodate patients with temporary difficulties.
  • A family discount may be available for one or more additional immediate family members who are undergoing or have completed orthodontic treatment at Theresa Shaver Orthodontics.
  • Multiple discounts for treatment (e.g., family discount and discounted insurance plans) are not available.

Payment Initiation

  • Fee assessment begins only when your or your child's active treatment program begins. If treatment is not recommended until a later date, Dr. Shaver will monitor your child's tooth eruption and growth patterns during these important years, but you will incur no charges for any recall visits or subsequent X-rays until active treatment is started.

What do we need from you to file your insurance claims?

We will need to see and make copies of your insurance card and valid driver's license. We will also need the insured person's name, contact information, birth date, social security number/subscriber identification number, and employer name and address.

How will my insurance company make its payment?

Most insurance companies make initial payments, followed by monthly or quarterly installments over the course of treatment. Payments are made directly to Theresa Shaver Orthodontics, P.C. We will work with your individual insurance carrier to determine the specific payment structure.

What happens if my employer changes insurance carriers?

Contact our office as soon as possible. You will need to provide us with the your new insurance information, including the insurance company's name, address and phone number, so we can help you file a new claim.

What happens if I lose my insurance coverage?

Contact our office immediately. The insurance company will no longer be sending payments on your behalf. You will be responsible for your remaining insurance balance.

What happens if I change jobs and get insurance from a different carrier?

Contact our office as soon as possible. We will work with you to verify insurance benefits from the new carrier. If you have orthodontic benefits, we will be happy to file with your new carrier.

What if I receive an insurance check at home made out to me, yet the payment should really go to Theresa Shaver Orthodontics, P.C.?

Please bring the check to our office and endorse it over to Theresa Shaver Orthodontics. If you have any questions or concerns about your insurance, please call our treatment coordinator. We're here to help and will gladly answer any of your questions and address any of your concerns.
